Greetings Real Power AME Church Family and Friends,
We are excited about the great return to our sanctuary! As we prepare to return, we will be hosting limited, in-person Sunday Services.
Registration is required for anyone who plans to attend in person.
DOORS OPEN 15 MINUTES BEFORE SERVICE START FOR REGISTRATION CHECK-IN AND TEMPERATURE CHECKS. Anyone with a temperature of 104 or above at check-in will be denied entry.
Real Power will continue to follow COVID-19 Safety Guidelines to keep everyone safe. MASKS MUST BE WORN AT ALL TIMES, and vaccinations are strongly recommended.

If you answer yes to any questions below, please do not attend worship service.
1. Fever?
2. Any shortness of breath or difficulties breathing?
3. Cough, sore throat, congestion, or runny nose?
4. Any flu-like symptoms, such as gastrointestinal upset, headache, or fatigue?
5. Loss of taste or smell?
6. In contact with any confirmed COVID-19 positive individuals?
7. Been out of the country in the last 14 days?
By completing this registration, you are acknowledging that you nor anyone you have been in contact with have not experienced any of the above symptoms in the last 14
days.
ASSUMPTION OF RISK. By completing this registration, I acknowledge and understand the following: 1. Attending and participating in services at Real Power AME Church includes possible exposure to COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; 2. I knowingly and freely assume all such risks related to COVID-19; 3. I hereby knowingly assume the risk of injury, harm, and loss associated with attending and participating in services at Real Power AME Church. I release, indemnify, and hold harmless the church, its pastors, officers, staff, members, volunteers, and representatives from any claims, demands, actions, and costs for any loss, injury, damage, or expense whatsoever that might arise out of attending and participating in services at Real Power AME Church.
